Saw palmetto (Serenoa repens) is one of the most-studied herbal supplements for benign prostatic hyperplasia (BPH), with a moderate evidence base. Early trials suggested benefit (Wilt 1998 meta-analysis) but larger and more rigorous trials showed less benefit (Barry 2011 CAMUS trial, JAMA, found saw palmetto 320-960mg/day equivalent to placebo for lower urinary tract symptoms). The current consensus is that saw palmetto has small effects on LUTS at 320mg/day of the standardized lipidosterolic extract, comparable to lifestyle modification but inferior to alpha-blockers or 5-alpha-reductase inhibitors for moderate-to-severe BPH. Drops format for saw palmetto is unusual — the evidence is primarily on the lipidosterolic extract (Permixon, Serenoa berry extract) delivered in capsules at 320mg/day. Vivalo does not disclose extraction ratio, standardization, or per-drop dose. Standard 50/100: real ingredient with moderate evidence for a narrow use case, held down by format uncertainty and no dose disclosure. Adding 'vitamin' to the name is misleading — no vitamins are disclosed.
