Sarsaparilla (Smilax spp.) is a traditional herb with a long history of medicinal use for skin conditions, syphilis (historically), and 'blood purification.' Modern RCT evidence for supplementation is essentially absent — the literature is dominated by in vitro sarsaponin studies and traditional-use accounts. PubMed searches for Smilax clinical trials in humans return few methodologically-adequate studies of Smilax-only interventions for the marketed indications. The 150 mg 20:1 extract dose (implying 3 g raw equivalent) is also low relative to traditional use (typical tincture or decoction preparations were consumed in larger quantities). Not a safety-based rejection — sarsaparilla is generally well-tolerated. Rejected on evidence and dose grounds.
