IV Drips: Worth It, or the Most-Hyped $200 You'll Ever Spend?

IV vitamin drips have become a booming wellness industry. Here's the medical reality: what the evidence actually says, when IV therapy is genuinely useful, and why the "Myers cocktail" you're paying $200 for at a spa mostly just makes expensive urine.

You've seen the ads. Feeling run down? Get a Myers cocktail. Hungover? Try the recovery drip. Long flight? Book the immunity boost. IV vitamin drips have gone from a niche medical intervention to a $2-3 billion consumer wellness industry, with lounges opening in every mid-sized American city and mobile IV services that come to your house.

The pitch is that vitamins delivered intravenously bypass the digestive system, providing 100% bioavailability and immediate cellular effects. The reality is that for a healthy person, most of what you pay for goes directly into your urine within a few hours, and the psychological effect of paying $200 and receiving medical-looking treatment is doing a lot of the work.

This post walks through what IV therapy actually does, when it's genuinely useful, when it's expensive theater, and the specific safety considerations that most spa lounges downplay.

What IV therapy actually is

Intravenous therapy involves placing a needle into a vein and infusing fluids and dissolved compounds directly into the bloodstream. In medicine, this is used for:

  • Rehydration in acute dehydration or GI illness.
  • Nutrient delivery when the GI tract cannot absorb (short bowel syndrome, severe malabsorption).
  • Emergency medication delivery.
  • Chemotherapy and other treatments requiring direct systemic access.

None of these are the wellness use case. The wellness IV drip is a mixture of saline solution plus various vitamins and minerals — usually B vitamins, vitamin C, magnesium, calcium, and sometimes glutathione, NAD+, or other trendy additions.

The most famous formula is the "Myers cocktail," originally developed by John Myers, a Baltimore physician, in the 1970s for chronic conditions like fibromyalgia and migraines. The original formula: magnesium, calcium, B vitamins, and vitamin C in saline. The evidence base for the original Myers cocktail is thin — a handful of small open-label studies with mixed results — and the formulation has been elaborated by wellness lounges into dozens of proprietary blends that mostly have no evidence at all.

What the evidence actually shows

1. For dehydration

IV saline definitely rehydrates you. So does drinking water and electrolytes. For non-severe dehydration in a healthy person, oral rehydration is as effective as IV rehydration in every published comparison. The WHO oral rehydration solution has been the standard for severe dehydration in developing-world medicine for fifty years and remains first-line even in cases severe enough to hospitalize.

You get IV fluids in the ER when you cannot keep oral fluids down (severe vomiting) or when your dehydration is severe enough that the volume replacement needs to be immediate. Neither describes your Sunday hangover.

2. For vitamin deficiency

If you are actually deficient in a vitamin or mineral, IV supplementation is a very fast way to correct the deficiency. It is also almost never necessary. Oral vitamins correct most deficiencies over 2-8 weeks, and the underlying cause of the deficiency should be addressed rather than papered over with periodic infusions.

The specific vitamins in most Myers cocktails and derivative formulas — B complex, C, magnesium — are all water-soluble or (in magnesium's case) have well-regulated homeostasis. If your blood levels are already normal (which they are for the vast majority of people paying $200 at wellness lounges), infusing more just raises the blood level briefly and then gets excreted.

Vitamin C is the specific example. Blood levels of vitamin C saturate at around 60-80 μmol/L. Above that, the kidney actively excretes vitamin C. An IV infusion of high-dose vitamin C temporarily raises blood levels above the saturation point — but within hours, the excess is peed out. There is no cellular storage benefit to the peak. Your cells are already saturated at normal dietary intake.

3. For hangover

Hangovers are caused by dehydration (partly), acetaldehyde buildup (a metabolic byproduct of alcohol), inflammatory cytokines, and disrupted sleep. IV fluids address the dehydration component. They do nothing for the other three. You may feel modestly better from the rehydration piece, but the same effect is available from drinking two glasses of water and an electrolyte packet.

The Myers-cocktail-for-hangover pitch was essentially invented as a wellness lounge upsell. The medical evidence for it is nonexistent.

4. For "immunity" and "energy"

Neither of these is a measurable outcome. "Boosting immunity" is not a coherent physiological concept in a healthy person. "Boosting energy" via a vitamin infusion has no meaningful mechanism in someone who is not deficient. If you are not deficient in the vitamins being infused, the effect is placebo plus the (real) pleasure of an hour of quiet time.

5. For specific conditions with actual evidence

A few IV therapies have real evidence bases for specific clinical conditions:

  • NAD+ infusions for certain neurodegenerative conditions and addiction recovery. Some early-stage evidence, still investigational.
  • Iron sucrose or iron carboxymaltose for iron deficiency anemia where oral iron fails. Real medical indication.
  • Vitamin K for coagulation correction in specific conditions.
  • Chelation therapy for confirmed heavy metal poisoning. Not for the vague "detox" claims most wellness lounges make.

These are legitimate medical uses. They are prescribed by physicians, administered in medical settings, and paid for by insurance when indicated. They are not the Sunday brunch drip.

The safety issues that get downplayed

IV therapy is not risk-free, even when administered by a trained nurse in a clean facility.

  • Infection. Any breach of skin with a needle carries infection risk. Bloodstream infections from IV catheters (bacteremia) are a documented complication.
  • Air embolism. Rare but potentially fatal. Air introduced into the venous system can travel to the lungs or brain.
  • Fluid overload. Rapid infusion of saline in patients with cardiac or renal disease can cause pulmonary edema.
  • Electrolyte disturbances. High-dose IV magnesium can cause cardiac arrhythmias. High-dose IV potassium can cause immediate cardiac arrest if infused too quickly.
  • Vein damage. Repeat IV placement in the same vein causes scarring, phlebitis, and eventual loss of vascular access.
  • Allergic reactions. Additives, preservatives, or vitamin B1 (thiamine) can cause anaphylaxis in rare cases.
  • Kidney strain. Massive infused loads of vitamins and minerals require kidney excretion. In patients with subclinical kidney dysfunction, this stresses the system.

Most wellness lounges use appropriate protocols and screen for major contraindications. Some do not. Mobile IV services in particular have variable oversight, and some operate in a regulatory gray zone.

The economics of the wellness lounge

The bag of saline itself costs about $2. The B-complex, C, and magnesium ingredients cost a few dollars more. The IV setup (tubing, cannula, alcohol swab) is under $10. Nurse labor for the 45-minute session is roughly $40-$60 in most markets. Total direct cost per session: perhaps $50-$75.

You are paying $150-$300. The gross margin is what pays for the couches, the plants, the receptionist, the marketing, the rent, and the profit. This is not necessarily unreasonable — the couches are nice, the experience is pleasant — but be clear about what you're buying. You are buying a spa experience with a medical topping. The medical component's contribution to your wellbeing is small.

The one legitimate case for wellness IV therapy

If you have a diagnosed condition where standard oral supplementation has failed — documented severe malabsorption, ongoing GI disease, or specific chronic conditions where IV therapy has evidence — there may be a case for periodic IV nutrient infusions under a physician's care. This is not the wellness lounge use case. This is the medical use case, and it should be prescribed and monitored.

For the vast majority of healthy people, the same nutrients delivered orally over a few days will produce the same physiological effect at 1-5% of the cost.

What to do instead

If you want to feel better, feel more energetic, or recover from a hangover, the evidence-based alternatives are cheap:

  1. For hydration: Drink water. If you're actually dehydrated (after exercise, illness, alcohol), an oral rehydration solution or a Liquid IV / DripDrop / LMNT packet gives you sodium, potassium, and glucose ratios that match what your gut can absorb quickly. This is the same physiological effect as IV saline for 5% of the cost.
  2. For vitamin deficiency: Get a blood panel. If you're actually deficient in something, take it orally at a therapeutic dose for the correction period. See a doctor if the deficiency is severe or recurrent.
  3. For fatigue: Sleep more. Get your fasting glucose, HbA1c, ferritin, TSH, and vitamin D checked. Most fatigue traces to one of those.
  4. For "immune support": Sleep, protein, moderate vitamin D if deficient, zinc lozenges at the first sign of a cold (there is actually decent evidence for this in shortening cold duration).
  5. For hangovers: Water with electrolytes, food with protein, ibuprofen for the inflammation piece if your stomach tolerates it. Time. Prevention (drinking less) is more effective than any post-hoc intervention.

The bigger point

IV wellness therapy is a category where the packaging (medical setting, nurse, needle) makes the intervention feel more effective than it is. Placebo responses to injections are much larger than placebo responses to oral supplements — this is well-documented in trial literature. The couch and the drip and the calm nurse are the therapy. The vitamins are the pretext.

None of this makes the industry categorically bad. If you find IV drips relaxing and the money is genuinely spare, doing them occasionally is roughly equivalent to a fancy massage. Just do it with clear eyes about what you are paying for.
